AGE 14

Your child should be attending their own IEPs by this age and participating. The Post-Secondary Transition Plans are created & your child needs to input their wants and needs. You can use the Symbol-Based Transition Interview or WiTransition Application for guidance.

JOB SERVICES

Contact your school’s designated Department of Vocational Rehabilitation (DVR) Counselor to sign up for DVR services. If you do not know who your DVR Counselor is, contact your child’s case manager or you can call Familiar yourself with the DVR Process and Choices in the Process and the DVR Program Guide.

AGE 17.5

LONG TERM FINANCIAL AND MEDICAL SUPPORT

Contact the Aging & Disability Resource Center (ADRC) of Milwaukee County - Call (414) 289-6660 or Email [email protected]. The ADRC will complete a functional assessment to determine long-term care funding.

DECISION MAKING CHOICES

Decide if guardianship, supported-decision making, or power of attorney is right for your child or your family. All people, including those with disabilities: Have the right to make decisions about things that affect their life. Can learn new skills to be more independent over time when they have the right support. Have the right to choose: who will provide their decision-making support, what types of decisions they want support to make, and how this support will be provided.

• What is the staff to client ratio?  Is it different if going out in the community?
• What does a daily schedule look like?
• What are your hours of operation and how long can a client stay during the day?
• Is there a minimum or maximum number of days/hours a client can be at the facility?
• What kind of training does your staff receive?
• Do clients bring their own lunches?
• Is there transportation available through the facility?
• Are there any therapy services provided onsite (physical/occupational therapy, art therapy, music therapy, animal therapy, etc.?
