Immunizations

Immunization Requirements

Wisconsin State law requires that every student who is enrolled in school must have evidence of receiving required immunizations or have a signed waiver on file in the school office. Immunizations may be available for West Allis residents at the West Allis Public Health Department. Clinic hours are Wednesday mornings and Thursday afternoons as well as by appointment. Vaccinations are one of the most important methods of protecting our children from diseases and the complications that can occur from these diseases. Vaccinations are especially important for school-aged children because children in school are regularly in close contact with others who may or may not be protected from these diseases.

We encourage you to make an appointment with your child’s health care provider to get your child vaccinated.

Encourage your child’s health care provider to use the Wisconsin Immunization Registry and any vaccination records you have for your child to determine which vaccines your child needs. If your child does not have a health care provider, please contact your local health department to determine if your child is eligible to receive vaccinations at the health department. Please be aware that your child’s vaccination records will be assessed to ensure that he/she is sufficiently vaccinated for his/her grade level or has a waiver on file. If sufficient documentation is not supplied for your child, he/she might be required to stay home from school. Legal Notice

Parents of students who are behind schedule or have no record receive the first Legal Notice and a Student Immunization Record by the 15th day the student is in school.

Parents of students who are behind schedule or have no record receive the second Legal Notice and a Student Immunization Record by the 25th day the student is in school.

Exclusion from School

Students have until the 30th school day to provide the appropriate documentation of immunization or a signed waiver. Starting on the 31st school day, noncompliant students in kindergarten through 5th grade must be excluded from school if the school is a public school and the school district’s compliance level from the previous school year was less than 99%.  

Kindergarten - 5th grade students that are not compliant with their immunizations must be excluded for a maximum of 10 school days unless they have received the vaccine or signed a waiver.  The Student Immunization Record includes the waiver in section 4.  (Waivers are good for one year.)
